The goal is to be a patient champion and provide options that guide toward informed decisions, better resources and better care options in your life and your death.
The On Your Terms philosophy is centered around personalized care that addresses the unique needs of each patient or caregiver. I believe in taking an integrative approach to healthcare challenges that considers the physical, emotional, and mental wellbeing of our patients and their families. My divine heart calling is dementia patients and those who are terminally ill. I see death as an important part of life and seek to find the beauty in this inevitable and highly personable human experience for my clients.

Shannon Pearson RN, BSN, MSCN, CHPN, PACS, BCPA, IN-NC, I am the founder and principal consultant for OYT. As an experienced nurse, educator, healthcare navigator, and patient advocate, I specialize in elder and dementia care as well as end of life consulting. I have a special passion for caregivers and the challenges in this important work. I am committed to helping patients and families clear away the clutter and confusion of the healthcare system, manage difficult situations and fill the "gaps" in healthcare, education, and resources.
My goal for you is to help facilitate creation of clarity, meaningful connection, and closure.
We offer 1:1 full circle healthcare navigation and patient advocacy services- whole person personal support and assessment, medication evaluation and education, offering actionable insights and recommendations- empowering you to make informed decisions, guidance in working within the healthcare systems, insurance issues assistance, attend medical appointments, lifestyle modifications and organization, home safety evaluation, and medical records procurement and organization.
A range of services- people with little to no support who need assistance with appointments, pre/post-surgery, transportation to/from, medication pick up, aftercare drop in's, etc.. If terminally ill, a solo client or solo ager might need assistance on many different levels. This is a flexible care offering based on personalized need. The aim is to help when a patient has unmet needs or requires support in a more short-term time frame. This focused assistance is provided in person, bedside and separate of the home health team.
Offering tailored assessments designed to meet your unique needs by dementia care certified nurse. In depth evaluation 0f your health history, lifestyle, and goals to create a personalized care plan. By identifying potential health risks early, we empower you to take charge of the long journey.
Caregivers have some of the highest stress, anxiety and burnout levels of anyone on the planet. It's crucial to develop and maintain effective coping strategies. Guidance provided for help, local resources, self-care and keeping your sanity while caring for your loved one.
While not providing legal advice, OYT can open conversations about care planning and end of life wishes helping individuals and their families understand options, ensure proper documentation and guide through the process to the right legal sources. Notary services will be offered in mid 2025.
